UCEAP Health Clearance Instructions

All UCI students studying abroad through a UCEAP program must be cleared for participation by a general healthcare provider and all specialists currently treating them for ongoing conditions. Please read the important points below and then follow the step-by-step instructions in the toggles to complete your UCEAP health clearance requirements.

Read these important points:

  • Students traveling to Botswana, Ghana, India, Senegal, Solomon Islands, South Africa, or Tanzania MUST obtain their health clearance from the UCI Student Health Center (UCI-SHC)*. 
  • Student traveling to all other countries may utilize either the UCI Student Health Center (UCI-SHC)* or a private physician. 
  • The health care provider and specialist(s) performing the clearance must be licensed in the U.S. and cannot be an immediate family member. (AMA Code of Ethics E-8.19).
  • Schedule your appointment(s) in plenty of time to meet YOUR program’s health clearance deadline. Students who do not complete this process will be withdrawn from their program. The deadline varies by program and is listed in your MyEAP portal under UCEAP Predeparture Requirements >> UCEAP Health Clearance.

*Feb. 2023 Update – Please be aware that UCI-SHC charges a $60 fee to complete administrative forms, which UC SHIP insurance does not cover.

  1. Schedule an appointment with your general healthcare provider and any specialists. Begin this process at least 3-4 months prior to your program start date.
    • For students obtaining health clearance from the UCI Student Health Center: Follow the instructions on UCI’s Student Health Center website under Health Clearance Requests, EAP/Study Abroad Requests. Note that you will have to upload the UCEAP Health Clearance Form into their portal. See “Before Your Appointment(s)” below for a link to this form.
    • Students traveling to Botswana, Ghana, India, Senegal, Solomon Islands, South Africa, or Tanzania MUST obtain their health clearance from the UCI-SHC.
  1. Complete, print, and sign the Confidential Health History Form.
    • Make a copy for your records.
    • Take a copy to your appointment(s) if required by your health care provider(s).
  2. Complete and print your portion of the Health Clearance Form.
    • Make a copy for your records.
    • Take a copy to your appointment(s) or upload into your health care provider’s portal.
  3. Complete and print the completion certificate for the UCEAP Online Health & Safety Course, linked in your MyEAP portal.
    • Follow the instructions to complete this online course in the MyEAP portal  and allow sufficient time for completion.
    • Take a printed copy of your completion certificate to your appointment(s), in case your healthcare provider asks for a copy.
  4. Research medication legality if you will be traveling with prescription medication. Discuss any concerns with your healthcare provider.
  5. Meet with any specialist(s) before you see your general practitioner so you can discuss medication and treatment plans.
  6. For students who obtain their health clearance from a private physician:
    • Review the UCEAP 2023 Annual Health Update for your host country.
    • Print any required forms or recommendations listed. Bring this information with you to your physician appointment.
    • Students who are going through a Kaiser provider must sign up for an appointment with their provider. Kaiser does not fill out the UCEAP Health Clearance form unless students attend an appointment with Kaiser.

All information is confidential and may only be shared with UCEAP officials in an emergency. Failure to provide complete and accurate information may be grounds for non-participation in UCEAP.

Take all the applicable forms to your appointment(s).

    • Confidential Health History Form – Give your provider a copy and keep the original.
    • Health Clearance Form – Ask your provider to follow the instructions carefully and make a copy of the completed form. Leave the copy with your provider and keep the completed original.
    • UCEAP Online Health & Safety Course completion certificate – Give to your provider if requested.
  1. Submit only the Health Clearance Form to UCEAP in one of the following ways:
    • eFax the completed, signed original Health Clearance Form to (805) 893 3021.
    • Email the completed, signed original Health Clearance Form to healthclearance@uceap.universityofcalifornia.edu.
      • Please note: Sending your completed Health Clearance Form via non-encrypted email is not private or secure. There is a possibility that your email could be intercepted and read by people whom you did not intend.

UCEAP must receive your Health Clearance Form by the appropriate deadline. The deadline varies by program and is listed in your MyEAP portal under UCEAP Predeparture Requirements >> UCEAP Health Clearance. Students who do not complete this process will be withdrawn from their program by UCEAP before the program start date.

  1. Review the Health chapter of the Guide to Study Abroad linked in your myEAP portal.
  2. Take the original Confidential Health History Form with you abroad in case of a medical emergency.

Students with Special Needs

Students with a disability or other chronic systemic condition for which they will seek accommodation abroad are advised to inform the Campus EAP office immediately so staff can notify the UCEAP Systemwide Office. The UC campus Disabled Students Office must send a memo to UCEAP indicating the nature of the student’s needs. In light of the varying conditions and services available, universities abroad may require this memo with sufficient notice for a fair evaluation of accommodation requests. The students must secure funding for the accommodation. Students who disclose needs at the last minute or require accommodations that cannot be made available in the host country may be advised to postpone participation or consider another site (NH 12/2013).
